What do I need to know?
Service description
The birth of a child must be reported to the registry office responsible for the place of birth.
If your child is born in a hospital or other facility that provides obstetric care, the birth will be registered by that facility. In this case, you should inquire at the maternity facility in good time which documents you need to bring with you on the date of delivery.
If the birth does not take place in a hospital or other facility that provides obstetric care (home birth), the birth must be reported in person to the registry office by a parent with custody within one week. If the parents are unable to report the birth, it must be reported by another person who was present at the birth.
We record every birth of a child within the city limits of Mainz in the birth registry of the Mainz Registry Office. We then issue the birth certificates.
A birth certificate includes:
- The child’s first name(s) and last name
- Gender
- Place and date of the child’s birth
- First names and last names of the parents
Hospitals automatically report a child’s birth to the registry office within 7 days of the birth. There is no deadline for parents to submit the documents to the registry office. However, we ask that you submit them as soon as possible. If your child was born at home, please go to “Report a birth outside a hospital” under “Related Services.”
Procedure
Depending on which hospital your child was born in, there are different procedures for registering the birth:
University Hospital
If your child is born at the University Hospital, you will receive an envelope containing the name declaration form and an information sheet. Please include the completed name declaration form with the necessary documents for the birth registration. Then drop off the envelope at the reception desk in Building 102 on the hospital grounds.
Marienhaus Clinic
If your child is born at the Marienhaus Clinic, you will receive a folder containing the name declaration and an information sheet. Please include the completed name declaration with the necessary documents for birth registration, which you have already copied. Please place the folder in the mailbox on the ward within 3 days.
The hospitals will prepare the birth notifications based on the submitted documents. The documents will be sent via courier service and will be received by the Registry Office within 7 days of your child’s birth. Provided the submitted documents are complete, we will register the birth within a few days.
If you have already submitted copies of your documents to the hospital in advance or have not yet submitted any documents to the hospital, please mail the original documents to us. Please send them to: Mainz Registry Office, Kaiserstraße 3–5, 55116 Mainz.
Alternatively, you can drop your envelope into the mailbox of the Birth Department in the foyer of the Kreyßig-Flügel wing of the City Hall Monday through Friday between 8:30 a.m. and 4:30 p.m. Please be sure to include your child’s name and date of birth so that we can identify your documents. The Registry Office will, of course, return the original documents you submitted to you.
Once all documents are complete, we can register the birth and send you the free birth certificates within a few days. If you need additional certificates subject to a fee (e.g., for the family register, standard or multilingual certificates), you can request and pay for them online. We will send you the requested certificates at a later date. Please use the online service “Order Birth Certificate” for this purpose.
We will notify you if any documents are missing or if it is absolutely necessary for you to visit us in person. To ensure we can contact you, please be sure to clearly write your phone number or email address on the name declaration form.

What do I need to bring or submit?
You will need the following documents:

For parents who are married to each other
- Parents' birth certificates
- Marriage certificate or a certified printout from the marriage register

For parents who are not married to each other
- Birth certificate of the mother

If paternity has already been acknowledged:
- Declaration of acknowledgment of paternity and the mother's declaration of consent
- Father's birth certificate
- Declaration of custody, if applicable
- Identity card, passport, or a recognized substitute travel document of the parents
- A birth certificate issued by a doctor, midwife, or obstetric nurse, provided they were present at the birth
A marriage certificate must also be submitted if the marriage has since been dissolved or the husband has died.
How much does it cost and how can I pay?
Fees
All parents receive three free birth certificates at the time of registration, to be used for applications for child allowance and parental allowance, as well as for health insurance purposes. In addition, you can request additional original birth certificates for your child when registering the birth. These are available in various formats:
- Standard Birth Certificate (DIN A4)
- Birth certificate for the family register (DIN A5)
- Multilingual birth certificate
- Birth registry extract
If you need additional birth certificates at a later date, you can pick them up at the archives or request them.
- One birth certificate: 15 euros
- Each additional certificate of the same type: 7.50 euros

- Cash payments and invoicing are not possible!
